This course is designed to provide individuals with the knowledge and skills to effectively investigate incidents and accidents in the workplace. This course covers topics such as incident investigation techniques, root cause analysis, and incident report writing. Participants will learn about the importance of incident investigation and how to gather and analyze data to determine the cause of an incident or accident. The course is intended for safety officers, supervisors, and anyone responsible for investigating incidents and accidents in the workplace. The information and skills gained from this course can be used to develop and implement effective safety programs and prevent future incidents and accidents. Instructional contact-time must be a minimum of 8 hours.

This course will offer participants a detailed understanding in various segments, which are not limited to:

Introduction to Investigating Incidents and Accidents: Overview of the importance of investigating incidents and accidents, types of incidents and accidents and their specific investigation considerations, roles and responsibilities of personnel involved in investigating incidents and accidents.
  2. Regulations and Standards: Understanding federal and international regulations and standards for investigating incidents and accidents,  Familiarizing with the compliance requirements for different types of incidents and accidents, identifying potential legal liabilities and liabilities in investigating incidents and accidents.
  3. Incident and Accident Analysis: Understanding the importance of incident and accident analysis in identifying causes and contributing factors, familiarizing with incident and accident analysis techniques, including root cause analysis and fault tree analysis, implementing incident and accident analysis procedures in the workplace.
  6. Report Writing: Understanding the importance of clear and concise report writing in incident and accident investigations,  familiarizing with the report writing requirements for different types of incidents and accident,  implementing report writing procedures for incident and accident investigations.
  7. Prevention and Recommendations:, Understanding the importance of identifying prevention measures and recommendations in incident and accident investigations, familiarizing with the requirements for developing and implementing prevention measures and recommendations, coordinating with relevant parties to implement prevention measures and recommendations.
  8. Auditing and Compliance: Understanding the importance of regular auditing and compliance checks, familiarizing with the auditing and compliance requirements for incident and accident investigations, implementing regular auditing and compliance procedures for incident and accident investigations.
  9. Conclusion: Review of key concepts and best practices for incident and accident investigations.

Training will be conducted via Microsoft Team Meeting, online. Meeting invites will be shared on the day before the first day of training.
